Social Prescribing Service

 

 

Neston and Willaston PCN have a Social Prescribing Link Worker Stephanie and a Wellbeing Co-Ordinator Amy, who are here to support you with a range of none medical related issues including loneliness and isolation, healthy living and other welfare issues.

Our dedicated team are able to work directly with you to enable you to have more control over your health and wellbeing in a way that works best for  you. Stephanie and Amy are able to signpost you to services and groups that are available within your local area for example Neston Angels a local befriending service.

This service is free and available to all of our registered patients.

If you would like to arrange a referral  to speak to either Stephanie or Amy please contact the reception team at your surgery or you can self refer by speaking to Neston Community Youth Centre to arrange an appointment.
